Directions:

2. Unzip it, and fill out the application in your word processor.

3. Save the completed application as a PDF file with your organization name in the filename. Example: "wise_little_league_app.pdf"

4. Don't forget to print off the signature page and sign it.  You can scan it and include it with your application, or mail it to us.
