REQUEST
for FUNDING - Instructions
GENERAL:
- The REQUEST for FUNDING is
almost paperless.
- Please download the
Microsoft Word Template or the Adobe PDF format file.
- Completed Requests for
Funding need to be returned to the United Way of Southwest
Virginia electronically via our e-mail address:

applications@swvaunitedway.org.
- Please name the file
containing your completed application as UW RFP "Agency"
2008. Example: for the Tri County Eye Clinic the file
name is UW RFP Tri County Eye Clinic 2008.
- Do not mail any paper to the
United Way office (except as noted below).
- Use the form as provided, as
any altered applications or files will be deleted from the
United Way system and will not be considered for funding.

PLEASE NOTE: The Deadline for Receiving United Way
Applications for Funding for 2009 is April 25, 2008. You
will be notified by Email that we have received your
application intact and in full by May 9, 2008. If there are
any problems with the transmittal of your application, we
will notify you at that time.
NOTE: There are only three pages that cannot be
accepted electronically. They are: Member Agency
Agreement (page 10), Counter Terrorism Compliance Measure
(page 11) and Agency Signature Sheet (page 12). Each agency
must mail a single (completed and signed) copy of
each of these to: United Way of Southwest Virginia,
P.O.B. 1225, Gate City, VA 24251
NOTE:
The original REQUEST for FUNDING must include county
specific information for all counties.
